INFORMED CONSENT AND ACKNOWLEDGEMENT - I hereby give my approval for my child’s participation in any and all activities prepared by Strategy To Results Consulting Group Ltd during the selected camp. In exchange for the acceptance of said child’s candidacy by Strategy To Results Consulting Group Ltd ., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Strategy To Results Consulting Group Ltd. and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions. In case of injury to said child, I hereby waive all claims against Strategy To Results Consulting Group Ltd . including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including basketball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

Media/Audio release I hereby voluntarily and without compensation authorize voice and video recording(s) to be made of me. I hereby give permission to GoFor10 to use, reproduce, and distribute the recordings, including my name and submitted photo(s). I hereby agree to release FGoFor10 from any claims, damages or liability arising out of such use or distribution. I understand that the use of my name, likeness, voice and video recordings will be primarily for the purposes of commercial use, education and/or promotion by this organization. I have read and understand the foregoing and I consent to the use of my picture and/or voice as specified for the above-described purpose(s). I further understand that no royalty, fee or other compensation of any character shall become payable to me.
